Fix Dialpad / Salesforce integration issues

First, let's disconnect and reconnect Salesforce.
It should be noted that in order to see the "Salesforce" integration for Dialpad, you need to click on any recent call in the bottom left of Dialpad:
1. If you see this error (or are not connected at all), try the following steps to see if it gets you back to good :-D
2. Click the three dots in the top right corner of the Salesforce plugin (ignoring that this image does not have the error message) ...
3. and then select "Disconnect".
4. Dialpad may blink, but then will switch to a plain Connect Salesforce link.
Click that link...
5. and it will pop up the Salesforce login. Log in with your user name and password.
6. and then enter your two-factor authentication code. You may need to switch to 1Password to grab that code if that's where you saved it. Then click Verify.
7. If, after switching back and forth between two recent calls/messages, Dialpad connects and removes the error, you're all set!
